Principles of personal data protection Pro Dialogu, z.s.

I. Who is the administrator

The administrator and at the same time the processor of personal data is the organization Pro Dialog, z.s., IČ: 22613421, kept at the Municipal Court in Prague under file number L 50 from 1 January 2014.

II. What data do we collect and process?

Personal data is any information relating to an identified or identifiable data subject. A data subject is considered to be determined or identifiable if he or she can be identified, directly or indirectly, on the basis of one or more elements specific to his or her physical, physiological, mental, economic, cultural or social identity. The data subject is understood here

• A natural person to whom personal data relates directly and is a client of the organization Pro Dialog, z.s., and of its own will has concluded a Cooperation Agreement with the organization Pro Dialog, z.s.;
• A natural person who is a party to the Agreement's legal representative (typically a child or children).

III. Scope of personal data processing

Pro Dialog, z.s., processes personal data to the following extent: a) identification data: name, surname, date of birth b) contact data: contact address, e-mail address, telephone number. In addition to this personal data, Pro Dialog, z.s., records and stores.

• so-called behavioral data concerning the participant of the Agreement, which are mainly e-mail communication with Pro Dialog, zs, participation in events of Pro Dialogu, zs, and other online and off-line activities that are part of fulfilling the purpose of the Agreement on cooperation with the organization Pro Dialog, zs (hereinafter referred to as the "Agreement");

• so-called sensitive data concerning a participant in the Agreement, which are in particular data on nationality, racial or ethnic origin, religion and philosophical beliefs, criminal convictions, health status and sexual life of the data subject, necessary for the purpose of the Agreement in accordance with authorization to perform SPOD;

(hereinafter referred to as "other data").

IV. Collection and processing of personal data

Pro Dialog, z.s., processes personal data in accordance with the applicable legislation, in particular Act No. 101/2000 Coll., On the protection of personal data, as amended. The participant may be asked to provide personal data during the first active contact with Pro Dialogem, z.s., off-line or on-line, during a personal or telephone request for cooperation (hereinafter referred to as the "first contact"). Pro Dialog, z.s., is entitled to collect, process and monitor personal data and other data of the data subject. Based on the Agreement and in accordance with legal conditions, personal data and other data concerning the participant of the Agreement may be transferred for processing to another administrator, typically SPOD bodies, courts (including guardians), PMS, etc.

V. Purpose of personal data processing

Personal data and other data to the above extent may be processed by Pro Dialogem, zs, for the purpose of fulfilling the purpose of the Agreement, typically for communication with the participant of the Agreement via email, telephone call, postal correspondence or sending SMS, to the extent: communication related with the status of a participant in the Agreement, in particular the sharing of records of activities under the Agreement; sharing third party information (provided, for example, by SPOD authorities, courts, lawyers, physicians and other professionals) that is relevant to the fulfillment of the purpose of the Agreement; informing about the activities of Pro Dialogu, z.s., etc.

VI. Consent to the processing of personal data

Consent to the processing of personal data in accordance with these Principles is granted by a participant in the Agreement to Pro Dialog, z.s., as the administrator of personal data at the first contact with him in accordance with Article IV. of these Principles.

VII. Time of consent

The participant of the Agreement grants the organization Pro Dialog, z.s, consent to the processing of personal data for an indefinite period, resp. until it withdraws its consent. Consent to the processing of personal data is voluntary. The data subject is entitled to revoke the consent at any time via standard communication channels. After revoking the consent, Pro Dialog, z.s., is obliged to ensure that all data of the data subject are deleted from the database and are not further processed.

VIII. Data subject access to personal data

From the moment of inclusion in the Database Pro Dialogu, z.s., the participant of the Agreement has the right to request information on the processing of his personal data at any time. In such a case, Pro Dialog, z.s., will provide the participant with information to the extent stipulated by Act No. 101/2000 Coll., On the protection of personal data, as amended.
